About Us

The Croft Practice cares for a mainly rural population covering an area of some 30 square miles in West Sussex, surrounding the Five Villages. As members of the Primary Health Care Team, we are committed to giving the best possible care and service to our patients. We provide medical services from our two main surgeries in Eastergate and Yapton.

Travel Plan for Patients Previously Attending Meadowcroft (Yapton)

How you can travel from Yapton to our new site at Eastergate Medical Centre, following the closure of Meadowcroft.

New Bus Route

  • 500 (Littlehampton to Chichester)
  • Operated by Stagecoach

The Stagecoach 500 bus service runs between Littlehampton, Yapton, Barnham, Eastergate and Chichester, offering a direct and convenient link for Yapton patients.

This route provides straightforward access to Barnham and Eastergate, with stops close to the medical centre.

New Zebra Crossing

  • Barnham Road

A new zebra crossing is being installed on Barnham Road to improve patient safety and access around the new site, particularly benefitting those arriving via the 500 bus route.

Other Bus Services

Additional bus routes operate between:

  • Yapton
  • Barnham
  • Eastergate / Westergate
  • Fontwell
  • Chichester

Registration

It is quick and easy to register with a GP using our simple online GP registration form. Whether you are new to the area or have never registered with a GP practice before, there is usually no need to come to the practice to complete your registration and no proof of address is required.

Online Access

You may use the NHS App to gain online access which will enable you to order repeat prescriptions, book appointments and print summary information from your medical record (this is currently limited to medications, allergies, and adverse reactions).

Alternatively, or if you do not have a smart phone or tablet, please contact the Practice to obtain a user name and password. This will allow you to link to TTP SystemOne Airmid in order to access SystmOnline.

Military Service Armed Forces Covenant

The GP Patient Survey is an independent survey run by Ipsos MORI on behalf of NHS England. The survey is sent out to over two million people across England. The results show how people feel about their GP practice.
