How Do I....
Obtain A Repeat Prescription?
Patients on regular medication will require repeat prescriptions and for a safer and speedier service, repeat prescriptions are computerised. Each time a repeat prescription is produced a new request form is also generated for you to order your next repeat prescription. Requests are not taken over the phone as this would block the telephone lines and errors can be made in verbal communication.
• Please allow two full working days for your prescription to be processed. This does not include Saturdays and Sundays.
• If you have more than one repeat prescription, please try to order all your items together.
• Tick which items you require on the request slip.
• Either deliver, post or fax your repeat prescription form to the surgery.
• We can dispense your medicines for you if you do not live within a one mile radius of a dispensing chemist.
• There are dispensaries at our two main surgeries in Eastergate and Yapton. Repeat medication can be ordered and collected from Walberton by arrangement.
• Repeat medicines dispensed at the surgery will be ready for collection from the afternoon two working days after the request is received.
• If we do not dispense for you, please clearly mark your repeat request if you wish your prescription to go to the Five Villages pharmacy in Barnham. Alternatively, we can keep your prescription at the surgery for collection, or we can post it if you enclose a stamped addressed envelope.
• If you require medicine which is not on the repeat request slip, please write your requirements clearly. Such medication can only be issued after confirmation by a doctor. If you no longer need medication which is on repeat, please ask a dispenser to remove it from your list.
• From time to time a note may appear on your repeat slip, advising you that your medication needs to be reviewed. You should then make a routine appointment to see a doctor.

Pre-Paid Certificates
It is sometimes worthwhile to buy a pre-payment certificate for the pre-payment of prescription charges. A single item prescription charge is £7.20.
You can get pre-payment certificates for three months or a year. The costs are as follows:
• A three month pre-paid certificate is £28.25
• A one year pre-paid certificate is £104.00
If you need regular repeat prescriptions, this keeps the cost down to a manageable amount during the year.
THIS FORM BELOW IS CURRENTLY DISABLED - PLEASE USE ONE OF THE ALTERNATIVE METHODS MENTIONED ABOVE TO REQUEST PRESCRIPTIONS.
