How Do I.... Register With The Practice?

Patients wishing to register with this practice will be requested to complete a questionnaire and may be required to attend a short health check with a healthcare assistant. This will only take a few minutes of your time, but it enables us to promptly record details of your medical history, as there is sometimes a considerable delay in the transfer of medical records.

Allocating a doctor

Although you will be officially registered with the practice, you may see any doctor. We do, however, recommend that you stay with one doctor through any one particular illness.

How Do I.... Change My Details?

If you change your name, address or telephone number please give full details, including your postcode, to reception. If you have changed your name it is necessary to provide written evidence. If you move outside the practice area, you will be asked to find a doctor in your new area. The Primary Care Agency on 01903 756800 will help you if you experience any difficulty.
