Practice Charter
When you attend the surgeries
You will be treated as an individual with courtesy, dignity and respect irrespective of your ethnic origin, religious beliefs, personal attributes or the nature of your health problems. A patient’s privacy will be respected at all times. Wherever possible, we will provide an area set aside for confidential conversation between patients and reception staff members.
Medical Records
Your medical records contain confidential information and will be kept so by the practice team. If you wish to see your medical records you should consult the practice manager. You have the right of access to medical records made since 1st November 1991 under the Access to Health Records Act 1990. A fee may be charged if you wish to have a copy of the record.
Confidentiality
We ask you for personal information so that you can receive appropriate care and treatment. This information is recorded on computer and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disciplinary team it is sometimes necessary that medical information about you is shared between members of the team.
Comments & Complaints
If you wish to comment or complain about any aspect of the practice, please contact the practice manager, Mrs Judith Walker. Any problem will be investigated in confidence and with sensitivity. We constantly look to improve services and patient care and will welcome any constructive comments which will help us do so.
Zero Tolerance
The practice operates a zero tolerance policy to aggressive and abusive behaviour. this includes any personal, abusive or aggressive gestures. Any incident of verbal abuse, whether in person or over the telephone, will be reported immediately to the practice manager and senior partner. Incidents are discussed by the partners, who will agree a course of action. This may result in the patient’s removal from the practice list.
Freedom Of Information
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available.
Private Medical Examinations
The doctors perform private medical examinations for insurance, pre-employment, driving etc. Please ask the receptionist for details of appointment times and fees.
